Thank you for submitting a yahrzeit plaque order form.
You can submit one form per plaque.
Each plaque costs $400 and will not be submitted to our vendor until full payment is received.

The form will be processed through the Admin Office at Herzl-Ner Tamid and in conjunction with our plaque vendors.

Please allow 7-10 weeks for the plaques to be processed, ordered and completed.
