SUMMARY:End of Life &  Advance Care Planning Workshop
DESCRIPTION:Sunday\, January 12\, 2020 • 5:30 PM – 8:30 PM • HNT\nCo-sponsored by Herzl-Ner Tamid\, Jewish Family Service Seattle\, and Kavana Cooperative \nDid you know that 80-90% of people say it’s important to talk with loved ones and put their wishes about end-of-life care in writing – yet less than 30% have actually done so? Sound familiar? If so\, this workshop is for you. Open to the HNT\, JFS\, and Kavana communities\, Julie Katz\, RN\, Julie Kohl\, MD\, and Jane Becker\, MD\, will create space for conversation – over dinner! – about palliative and end of life care\, helping you clarify what you want at the end of life. We will then discuss advance care planning\, specifically walking you through how to complete advance directive documents that are legally valid in Washington State. You will leave with the knowledge and materials needed to name a health care power of attorney and complete an advanced directive. \nSpace is limited. Babysitting may be provided upon request to Rebecca@h-nt.org. Register at h-nt.org/advance-care by January 5 so that we can prepare materials and meals in advance. \nAll materials\, vegetarian dinner\, and any babysitting are free of charge\, thanks to a grant from the UW Cambia Palliative Care Center of Excellence and the Jewish Federation of Greater Seattle.
